Residents who experienced flooding can report residential sewer issues here: http://www.berwyn-il.gov/Departments/PublicWorks/RequestforInformationonResidentialSewers.aspx

Please note that this is an incident report for a single occurrence of flooding. You will need to complete a report for each occurrence.  Filling out this report is important because the data will help the city communicate the urgency to the Chicago Metropolitan Water Reclamation District, as well as help the city identify the areas with the greatest need and set priorities.

Currently, this survey is only online. If you have a friend or neighbor without a computer/internet access, consider offering them an assist by sharing your computer and internet so they can report their flooding experience. Or explain to them that they can use the library's computers to submit the survey.
